What We Do

The John Muir/Mt. Diablo Community Health Fund supports health initiatives that address current and emerging health care needs. To do so, we distribute grants to and partner with the leaders of these initiatives: community-based, nonprofit organizations that provide either high quality, affordable primary, specialty, dental, and behavioral health care or innovative wellness and support programs that contribute to good health.

We define a health initiative as something that increases access to and closes gaps in existing services and is substantial and sustainable, because it includes:

  • An innovative vision for change
  • A well-crafted implementation strategy
  • A professional evaluation

Our Funding Process

Our unique funding process does not start with a proposal. Instead, we engage in a series of discussions to identify visionary leadership, strategies, and key players. Then we tailor a timetable for discussions and actions that lead to formulating and funding a health initiative.
