Current Health Initiatives

A health initiative increases access to and closes gaps in existing services. It is substantial and sustainable, because it includes:

• An innovative vision for change
• A well-crafted implementation strategy
• A professional evaluation

Over the years, our most valuable initiatives – our legacies – have been those where our funds have helped organizations initiate or expand health care services that continue to improve and maintain the health status of uninsured, underinsured and underserved people in Central and East Contra Costa County, long after our funding period has ended.

Today, we aim to create similar, enduring initiatives by supporting programs that expand, enhance, and integrate primary care, specialty care, dental care, and behavioral health care, as well as those that spark systems change that can lead to greater efficiencies in delivering health care services for the underserved.
