In 2009, Dr. Roy Kaplan championed an interdisciplinary group to develop a single cross campus short list of critical lab results and a corresponding list of a second tier of abnormal results known as “clinically significant results”.  The work group, which included representatives from nursing, medical staff and laboratory, developed new operational definitions of critical results and clinically significant results, and agreed upon timeframes for phone notification of these abnormal lab results.  The culmination of this work resulted in a new cross campus "Critical Results and Clinically Significant Result Reporting" policy, which was approved by the Medical Executive Committees on both campuses and subsequently by the John Muir Health Board of Directors in February 2011.

“Critical Results” are those test results that fall significantly outside the normal (reference) range and may be imminently life threatening or lead to significant morbidity if not addressed in a prompt manner.  For hospital inpatients, these results are reported by phone to the RN caring for the patient or Charge Nurse immediately (within 15 minutes).  For outpatients, “Critical Results” are reported by phone from MuirLab to the ordering clinician or authorized agent/office staff as soon as possible (< 60 minutes).

“Clinically Significant Results” are those test results that fall outside the reference range but are not at the critical level. Medical staff has determined that phone call notification of the result, as a courtesy, is in the best interest of patient care and safety.  Several previously defined critical results now fall into this category.  For both inpatients and outpatients, “Clinically Significant Results” are reported by phone to the RN (inpatient) or ordering clinician or authorized agent/office staff within several hours (< 8 hours) from the time the result is verified/finalized.  For outpatients only, these calls are deferred between 10:00 pm and 7:00 am.
