All health care providers in the US, who administer to any child or adult, diphtheria, tetanus, pertussis, measles, mumps, rubella, polio, hepatitis A, hepatitis B, Haemophilus influenzae type b (Hib), trivalent influenza, pneumococcal conjugate, meningococcal, rotavirus, HPV, or varicella (chickenpox) vaccines are required under the National Childhood Vaccine Injury Act (42 USC §300aa-26) to provide, prior to administering each dose of the vaccine, the person to be vaccinated or their guardian a copy to keep of the relevant current issue Vaccine Information Statement (VIS) produced by the CDC.

Besides, the name and title of the individual who administered the vaccine, date of administration, vaccine manufacturer and lot number, notations in the person’s medical record should include the edition date of the VIS, and the date the VIS was provided.
