The John Muir Community Health Alliance, an integral part of John Muir Health, was created to assist the community in achieving optimal health through education, collaboration, and health and wellness services. John Muir Community Health Alliance's goal is to improve the health status of communities in John Muir Health's service area by helping to create, link, and support community-based partnerships that address unmet needs of residents in central and eastern Contra Costa County. Collaborative partners include private and public health and social service agencies, community-based organizations, and other providers. Current collaborative health initiatives include the Monument Community Partnership, the Mobile Health Clinic and Senior Services.
