Inpatient Diabetes Educator - Concord

"The John Muir Diabetes Center in Concord is one of the few places patients can still get the individualized attention and encouragement for their progress, however great or small. I'm so grateful to work in such a supportive environment, which allows educators to truly make a difference in someone's life. Working the Diabetes Center makes me a better person; I feel we get a lot more out of our work with patients and the community than we give. The Diabetes Center provides Diabetes Self Management Education in an outpatient setting. Services include insulin pump education, individual and group classes, support groups, and continuous glucose monitoring. The RN staff also assists with education and management of inpatients. The Center works collaboratively with registered dieticians and counselors to address the education and behavioral changes required for patients to better manage their diabetes.

All the RNs and RDs are Certified Diabetes Educators. Experienced RNs support staff development through the RN Residency Program and focused inservices. Through interdisciplinary coordination of patient education, the Diabetes Center meets or exceeds several quality benchmarks...it is both American Diabetes Center recognized and Joint Commission Certified for Disease Specific Care.
