John Muir Home Health Services is both Medicare certified and Joint Commission accredited, and is a long standing member of NAHC and CAHSAH. Our Agency provides in home services to patients from newborns to seniors that reside in Contra Costa and parts of Alameda and Solano counties.

John Muir Home Health's multidisciplinary team of experienced RNs, Physical, Occupational and Speech Therapists; Medical Social Workers; Dietitians and Certified Home Health Aides collaborates to provide intermittent, short term, skilled care to patients under the direction of the physician.

Our Home Health team works with the patient, their families and their physician to develop a plan to provide the best care possible in the patient's own home by assessing their specific needs for education and care to help them recover.

John Muir Home Health participates in a national Home Health comparison survey, called Home Health Compare. Scores are posted quarterly on www.medicare.gov/HHCompare/Home John Muir Home Health consistently compares favorably when compared to other Home Health agencies.

Home Health does not employ new graduates, as Title XXII requires the professional Home Health Nurse to have a minimum of one year of acute care experience. Previous Home Health experience is preferred but not required. The ideal Home Health nurse is highly organized, has strong critical thinking skills and solid assessment abilities, and prefers working in a setting that requires a high degree of autonomy. They communicate clearly, concisely and have computer proficiency. They are good listeners and enjoy being part of a team working for outstanding patient outcomes.

Home Health nurses are generalists. Our Agency does on-the-job training for assessment, evaluation, patient education and care delivery for a variety of diagnoses prevalent in our population

Schedules are patient driven but allow the professional nurse the maximum amount of flexibility. State-of-the-art point-of-care service data entry and patient information retrieval allow for effective use of care provider time.
