2009 YEAR-TO-DATE COMMUNITY BENEFIT REPORT,
MARCH 2009

For 2009, it is projected that the Community Health Fund will have $2.3 million available for granting. From January through March, grants totaling $1,191,776 have been approved. The Board expects to consider another round of grants totaling close to $1 million at mid-year. The current grants approved are listed below:

GRANTS APPROVED FOR HEALTH PARTNERSHIPS: Grants to Create or Implement Action Plans that Expand & Enhance Delivery of Health Care
La Clinica de La Raza: Behavioral Health Integration Project Integrating behavioral health with primary care in the Monument and Pittsburg clinics	$	288,881
Center for Human Development: African-American Health Initiative Planning Collaborative: Center for Human Development, La Clinica de La Raza, Planned Parenthood, County Health Services, African-American Residents in Bay Point & Pittsburg	$	152,895
La Clinica de La Raza: Monument Clinic Funding for renovating the building purchased this year that will be the permanent and greatly expanded home for the Monument Clinic in Concord	$	750,000
FORE & La Clinica de La Raza: Latino Osteoporosis Initiative Implementation Collaborative: Foundation for Osteoporosis Research & Education (FORE), La Clinica de La Raza, Latino Consultants, CA Hispanic Osteoporosis Foundation & Procter & Gamble
La Clinica de La Raza: Medical & Social Services for Aging Latinos Making primary care and the clinics in the Monument and Pittsburg more aging-friendly and enhancing support services for seniors
