All durable medical equipment (DME) requires prior authorization. In order to ensure that DME is being obtained from contracted providers, before accepting equipment, please have your physician contact the John Muir Physician Network for authorization. The health plans are responsible for payment related to DME items and will not pay if items are obtained without authorization or from a non-contracted vendor.

• Tens Units
• Traction Units
• Muscle Stimulators
• Orthotics & Prosthetics
• Glucometers
• Insulin Pump & Supplies
• Hearing Aids
• Ostomy, Urological, Trach Supplies
• Mastectomy Bras & Prosthetics
• Continuous Passive Motion Rentals - coverage based on health plan benefit (have your physician contact the John Muir Physician Network to confirm coverage)

NOTE: Equipment provided by your physician may not be covered by your health plan. Prior to accepting equipment from your physician, ask them to verify coverage by contacting the John Muir Physician Network.
