Service Commitment

The Mt. Diablo Laboratory at John Muir Medical Center, Concord Campus is committed to the value "Patients Come First."

We carry out this commitment by supporting our clients needs and being responsive to input from both patients and laboratory users. Continuous quality improvement is practiced to evolve an ever-improving organization based upon internal and external comments and national benchmarks.

Scope of Service

The Laboratory provides service 24 hours a day, 7 days a week for adult and pediatric inpatients and stat outpatients. Routine outpatient service is provided through the Laboratory Service Centers.

The Pathology Department is under the direction of pathologists who are board certified in Clinical and Anatomic Pathology.

Service is provided by California State licensed clinical laboratory scientists, laboratory assistants, phlebotomists, and histologists.

Service is provided at the following sites: inpatient nursing units and critical care units; emergency department, and multiple outpatient service centers.

Activities performed include: immunohematology (transfusion service), immunology, hematology, urinalysis, coagulation, microbiology, chemistry, therapeutic drug monitoring, referral of specimens to outside reference laboratories, specimen collection (phlebotomy), point of care testing oversight, anatomic pathology, and cytopathology.

Specimen Pick Up

Specimens are picked up by our courier Monday through Friday and taken to John Muir Medical Center, Concord Campus for processing. Specimens are analyzed in the John Muir Medical Center, Concord Campus Laboratory soon after arrival. Reports are generated periodically throughout the day. Upon request, we will telephone or fax reports to your office.

Specimens will be picked up within the local area. If you are outside of the hospital area and would like to arrange to have pick-up service, please contact the medical center's Service Representatives at (925) 674-2188.

Clinical Trials Support

John Muir Medical Center, Concord Campus Laboratory supports clinical trials by providing testing under the special circumstances required by the clinical trial. Results of the testing can be transmitted to the researcher via several different ways.

One month advance notice is requested to work through the details of the laboratory requirements of the clinical trial, to educate the staff in what is expected of them, and to establish the billing procedure.

Medicare Billing Information

John Muir Medical Center, Concord Campus Laboratory has computerized checking of Medicare outpatient test orders versus ICD-9 codes. When the test(s) are not considered medically necessary by the Medicare Program for the ICD-9 codes given on the lab requisition, an Advance Beneficiary Notice (ABN) will be generated for the patient to sign accepting financial responsibility.

The ordering physician has the responsibility for determination of medical necessity for each lab test ordered. You must provide diagnosis code(s) (ICD-9) supporting medical necessity for each test billed to Medicare, MediCal, or other federal programs. Multiple ICD-9 codes are generally necessary if you order multiple lab tests.

If you order a test for which Medicare will likely deny the lab payment, you are required to inform the patient and have him sign an ABN agreeing to financial responsibility. The patient should bring the signed ABN with the lab requisition to the outpatient service center.

Fill tubes to their vacuum fill volume for accurate test results. Check on tube that the expiration date has not been exceeded.

For frozen specimens being held until delivery to the laboratory, self-defrosting freezers are not adequate.
