Each patient is asked to participate in developing a discharge plan that supports ongoing stability and growth. The Social Services staff coordinates referrals to the next level of care, ensures that the patient will return to a safe living environment, and involves family and significant social supports in the plan. Every patient is provided a follow-up appoint with either their own provider or a new provider as appropriate at time of discharge. This appointment is scheduled within 7 days of discharge 80% of the time.
The Social Service staff assists patients in stepping down to lower levels of care, either in structured programs that begin immediately or in outpatient psychiatry and therapy sessions within seven days of discharge.
Family therapy sessions are conducted during the patient's admission to provide support and education, resolve barriers to family stability, and discuss options for post-hospitalization care.
Every patient is screened for dietary concerns upon admission and high risk patients receive a follow-up visit by our dietitian and are monitored by our dietary assistant.
Patient confidentiality is very important to all staff at the Behavioral Health Center. It is certain that only those who need to know (ie. Staff directly involved in the treatment) access patient information which includes knowledge of hospitalization. Employees of John Muir Health can be assured that their presence and access to information is held in confidence and we maintain separate data bases and records to ensure this. Only the patient can authorize information be shared outside the organization and this is done in writing, with a few exceptions that are explained during admission.
