Q: What is the admission procedure?

The John Muir Physical Rehabilitation Center accepts referrals from hospital case managers, physicians and insurance case managers. A licensed clinician / referral coordinator evaluates patient for admission. This service is free of charge. The referral coordinator can be reached at (925) 947-5252.

Q: What are the admission criteria?

  • Appropriate rehab diagnosis needing two or more disciplines such as physical therapy, occupational therapy and speech therapy.
  • Medically stable but requires continued medical monitoring and twenty four hour rehab nursing.
  • Adequate endurance for three hours of therapy a day.
  • Cognitive and behavioral issues will not preclude participation and progress.
  • Planned /anticipated discharge to other than Skilled Nursing Facility, i.e. home, relativeÕs home, residential living program, board and care.
