At John Muir Health, we believe that a good patient-doctor relationship provides the best foundation for a proactive approach to your health. Your primary care physician is your first point of contact for nearly all health-related matters, so it’s important that you find a doctor you trust and respect—and, most importantly, who respects you.  

The health plan open enrollment period presents an ideal opportunity to select a great doctor located conveniently close to where you live or work. As you sort through your choices for a health insurance plan, you’ll want to make sure the plan you select allows access to a John Muir Health doctor and our full range of services and facilities. Our Open Enrollment Resource Center will help you navigate the process and make the best possible health care choices for you and your family.

Step One: Find a Doctor

With over 280 primary care physicians in our network, John Muir Health has the right doctor for you. Use our Find a Doctor tool to narrow your search based on location or other factors that are important to you. Many of our doctors have videos that introduce you to them and their practice. You can send the results of your search by email or print the list of a few doctors you like just in case your first choice is not covered by your plan.

Step Two: Find the Correct Provider ID

To ensure access to the broad range of John Muir Health services, including over 600 specialists, highly-ranked hospitals, Urgent Care Centers, a variety of medical services, and health education and wellness programs, you enter the doctor’s John Muir Physician Network Provider ID when enrolling. Many doctors belong to multiple medical groups, making it easy to select the wrong medical group provider ID.

To receive instructions on how to locate your selected doctor’s John Muir Physician Network Provider ID on some of the major health plan websites, select your plan (if shown) from the list below. You may also download the John Muir Physician Network Provider Directory for easy reference.

This is just a sample for reference, and not a complete list of the health plans we accept. Please check our list of accepted health plans. However, even if your plan is not listed, it’s still a good idea to check directly with your health plan administrator to see if the John Muir Physician Network is part of their plan.

Switching to John Muir Health When You Already Have an HMO

If you're already enrolled in an HMO plan and wish to change your medical group to the John Muir Physician Network, simply call your health plan's customer service department at the number on the back of your health plan ID card and ask to be assigned to a doctor affiliated with John Muir Physician Network.

Preferred Provider Organization (PPO), Health Savings Accounts (HSA)-eligible High Deductible Health Plan, and Point of Service (POS) products:

You can select any John Muir Health doctor. Simply follow these steps:

  1. Use our Find a Doctor tool to select the doctor that’s right for you. Search by name or specialty, and view video profiles.
  2. Call the doctor's office to confirm he is accepting new patients and belongs to the health plan you’ve chosen, and make your first appointment.
