La Clínica de La Raza: Contra Costa Health Information Technology Initiative
Coordination and continuity of care across health care settings is a critical element in improving the health of underserved populations – and improving the efficiency and reducing the cost of that care.
Because electronic health records can facilitate this coordination and continuity, in 2013 the John Muir/Mt. Diablo Community Health Fund (CHF) provided a $400,000 grant to La Clínica de La Raza to add an electronic record-keeping component (electronic dental record) and digital x-ray system for its Contra Costa dental clinics. La Clínica integrated the electronic dental record into its agency-wide electronic health record system.
After completion of the electronic dental record implementation in June 2014, the CHF approved a $300,000 grant to La Clínica for a second phase to implement the NextGen Electronic Medical Record (EMR) system at La Clínica’s Concord and Pittsburg clinics.
This initiative supports La Clínica’s strategy of strengthening the standardization and coordination of its medical, dental, optometry, behavioral health, and health education services. This second phase will enable La Clínica to:
- Improve clinical outcomes
- Reduce medical errors
- Improve financial and operational performance
- Make the organization compliant with federal regulatory requirements in the Affordable Care Act
