How to Schedule/Refer
PATIENTS: To schedule with a specialist, please call (510) 985-5200. To schedule with Cancer Services, please call (510) 420-8000.
PROVIDERS: To refer a patient, please fax the following information to the desired fax number listed below:
- Referral Form
- Insurance Authorization (if required)
- Copy of insurance card, front & back
- Any relevant clinic or hospitalization notes, diagnostic test results and imaging
- Patient’s daytime contact information

Payment Options
Health Insurance
Depending on your plan details, your health insurance plan may not cover part or all of your treatment costs. Please contact your health insurance plan for questions about what’s covered and how much your co-pay amount will be. Please note that we collect Urgent Care co-pay amounts at the time of service.
Self-Pay
If you’re planning to pay for your visit yourself, the office staff will collect the full amount due at the time of your appointment. We offer several convenient ways to pay for your visit, including:
