Who We Are
John Muir Home Health Services provides assessment, evaluation, education and treatment in the comfort and safety of the patient's place of residence. Services are delivered per your physician's order and include skilled nursing, physical, occupational, and speech therapy; medical social work and certified home health aide/personal care.
Who We Serve
Home Health is a physician-ordered service for patients who are homebound and have a skilled need. An exception to the homebound status is the Maternal Child Home Visit Program which is a unique service that examines newborn infants and provides information/instruction to the family. Assessment, care and community resources are made available to the new mothers as well.
Services are provided to individuals of all ages, from infants to seniors who reside in Contra Costa and parts of Alameda and Solano counties. The range of services and care provided include those who have certain clinical needs in the areas of cardiac, respiratory,wound/ostomy, diabetes, pediatric and medical/surgical care.
Memberships and Accreditations
John Muir Home Health is a Medicare-certified, Joint Commission-accredited agency and is a longstanding member of the National Association of Home Care and the California Association of Health Care at Home.
The Home Health Team
The John Muir Home Health team are employees of John Muir Health. We are a multidisciplinary team of experienced and caring professionals including:
- Registered Nurses
- Physical, Occupational and Speech Therapists,
- Medical Social Workers,
- Dieticians
- Certified Home Health Aides
who collaboratively work together to help the patient achieve theirhealth care goals.
Our Home Health team works with the patient, their families and their physician to develop a plan to provide the best care possible in the patient's own home.
Each discipline works very closely with patients and families to assess the specific needs for education and care to help them recover.
Quality
John Muir Home Health works closely with Lumetra, a Quality Improvement Organization. The Agency participates in a national home health comparison survey, called Home Health Compare. John Muir Home Health consistently compares favorably when compared to other Home Health agencies.
Home Health staff enjoy a great deal of autonomy.
Schedules are patient driven but allow the professional clinician the maximum amount of flexibility. State of the art point of care service data entry and patient information retrieval allows for effective use of care provider time.
