Request a Medical Record

To access or receive copies of your medical record or authorize John Muir Health to release your medical information to another facility or individual, you must submit your request with a signed authorization form to the Health Information Management Department (medical records) or the physician practice location where your services were provided.

Unless you are using the record for continuity of care, we may charge a fee for the costs of copying, mailing or other supplies associated with your request. Upon receiving your request, we will inform you of the fee.

To submit your request, fax, mail or deliver the appropriate signed authorization form to the location where services were rendered:
