Building Bridges to Better Health
Model Past Partnerships
Though the Community Health Fund's financial support for some of our past partnerships has expired, we include them here because they exemplify our belief in collaboration and the types of lasting change it fosters. In different ways, these programs remain active because they work – they improve health and access to health care in some of Contra Costa County's most underserved communities – and because they were developed in way that was affordable and financially sustainable. We are proud to have helped catalyze the birth and growth of these initiatives and to be associated with the individuals and organizations that have made them successful.
