The John Muir Physician Network serves over 80,000 members. You too can join the Network.

• You can switch your group at any time by calling your health plan directly and requesting that you be assigned to the John Muir Physician Network.
• If you are new to your insurance plan or completing an open enrollment form, indicate you want to be assigned to the John Muir Physician Network.
• If it is not your open enrollment period, you may contact your health plan's customer service department (phone number should be on the back or your ID card) and request that you be assigned to the John Muir Physician Network.
