Important Considerations for Open Enrollment

Open enrollment is a period of time in which eligible subscribers may elect to enroll in, or transfer between, health insurance plans offered by their employer, union or other sponsoring entity. This is a time when you can join the John Muir Physician Network if it is one of your options.

At the John Muir Physician Network there are more than 800 primary care physicians and specialists. Affiliation with a John Muir Physician Network physician means you have access to John Muir Medical Center, Walnut Creek Campus and John Muir Medical Center, Concord Campus as well as hospitals in San Ramon and Antioch, conveniently located outpatient lab and x-ray facilities including the new John Muir Outpatient Center, Brentwood outpatient facility, urgent care centers and educational seminars and health screenings.

What is the John Muir Physician Network?

The John Muir Physician Network and its physicians are leaders in providing high quality health care. More than 75,000 HMO members are signed up with the John Muir Physician Network. We have contracts with seven HMO plans: Aetna, Blue Cross, Blue Shield, Cigna, Health Net, Health Net Seniority Plus and PacifiCare. The John Muir Physician Network physicians have many PPO contracts including Network Select (EPO) and most of them accept Medicare.

The John Muir Physician Network and its physicians are leaders in providing high-quality health care. A recent statewide survey of HMO members ranked the John Muir Physician Network first overall among San Francisco-East Bay medical groups. The John Muir Physician Network also ranked in the top 10% of medical groups throughout California on:

Important information to consider as you are completing forms for your employer.

• To complete enrollment forms for your employer, you will need to use your health plan's directory/web site for the correct physician code and/or medical group number.
• Be sure you are signing up for your primary care physician through the John Muir Physician Network. A few primary care physicians belong to more than one John Muir Medical Group/Medical Group, and will have different numbers or codes for each group after the doctor's name. If there is more than one code, choose the one for John Muir Physician Network.
• Confirming your John Muir Physician Network membership.

When you receive your enrollment confirmation, look carefully for the John Muir Physician Network's name and medical group number. The health plan may assign you to the primary care physician you requested, but to a different medical group. Be sure that your medical group is the John Muir Physician Network so you can access our hospitals, local specialists and services. Check your card and papers for our medical group name and number, and call the health plan if it is not what you expected.

How to sign up with a primary care physician if you have PPO, standard Medicare, and non-HMO insurances.

Find a physician following the steps under HMO insurance. For PPO Plans, you do not need to indicate the primary care physician's name on the forms you complete for your employer. Call the primary care physician's office directly for an appointment. Some primary care physicians have full practices, so have several names in mind. If you have any problems making an appointment, call Customer Service at (925) 952-2887 and tell them you have PPO or Medicare insurance. They will make every effort to assist you.
