The following are some of the more frequently asked questions that come into our Customer Service department. If you are unable to find what you are looking for, or for additional assistance, please contact our Customer Service department at (925) 952-2887 or send us an email. A: The John Muir Physician Network was formed in 1996 and is a non-profit California medical foundation. The John Muir Physician Network has over 800 physicians and is part of John Muir Health. Through this network of 800 providers, we serve more than 80,000 members.
A: IPA stands for Independent Practice Association. Muir Medical Group IPA is a group of independent physicians who have joined together to provide medical services to HMO and other health care company patients. Physicians belonging to Muir Medical Group IPA usually have their own offices, separate from other physicians in the John Muir Medical Group.
A: Your HMO provides us with your eligibility and benefit information. While this enrollment process can take up to 90 days, please be assured that your benefits are effective from your date of eligibility.
A: Your health plan is responsible for your demographic and benefit information. You must contact your employer or health plan directly in order to change this information. Your health plan will notify the John Muir Physician Network with any changes.
A: Insurance cards are issued by your health plan. Please contact them directly.
A: All premiums are handled by your health plan. Please contact them directly at the number on the back of your insurance card.
A: You will need to contact your health plan. Once they have updated your enrollment record, they will provide that information to us. Additionally, please make sure to mention that you wish to remain affiliated with the John Muir Physician Network. Some Muir Medical Group IPA primary care physicians belong to multiple IPAs and the health plan may inadvertently switch you to another IPA.
A: Our laboratory provider is MuirLab. You will need a form from your physician stating the lab tests being requested. To find the location closest to you, click here.
A: For any life-threatening emergency, call 911 or go directly to the nearest hospital. This does not require prior medical review. However, please inform your primary care physician and health plan within 48 hours of your visit. If you use the emergency room in a non-emergency situation, it is possible you will be financially responsible for these services.
A: A primary care physician (PCP) is your first, or primary, source of medical care—your personal doctor. The PCP you choose coordinates your medical care, including checkups, referrals to specialists, lab and x-ray services and hospital admissions. PCP-based care is an effective way to maintain better health: regular physician visits help build strong, secure doctor-patient relationships. When you enroll with the John Muir Physician Network, you choose a PCP for your health.
A: You can switch your group at any time by calling your health plan directly and requesting that you be assigned to the John Muir Physician Network.
A: If you are new to your insurance plan or completing open enrollment forms, indicate your choice on your enrollment form. If you are not changing plans, you may contact your health plan's customer service department listed on on the back of your identification card. Your health plan also may require you to select a PCP. A: John Muir Physician Network physicians use John Muir Medical Center, Walnut Creek Campus and John Muir Medical Center, Concord Campus, as well as other local hospitals.
