Medical Records Requests

Requesting medical records:

Individuals who have been a patient or currently are a patient at John Muir Medical Center^SM-Walnut Creek Campus or John Muir Medical Center-Concord Campus have a right to access or receive copies of their medical records except under certain very limited conditions. A written request is required.

The ROI Authorization form authorizes John Muir Health to disclose personal health information (PHI) to the patient, their legal representative or designated entity.

Please Download and complete the attached Authorization for Use/Disclosure of PHI form or send your written request as follows:

How to Obtain a Copy of a Birth Certificate

Request for birth certificates should be directed to the Contra Costa County Administrative Offices for Records at Contra Costa County Office of Records at (925) 335-7903.
