Protecting Your Health Information
John Muir Health has always vigorously protected our patients' confidentiality in the way we use and disclose personal health information (PHI). Even stricter policies have recently been enacted that are aligned with new regulations from the Health Insurance Portability and Accountability Act (HIPAA) and the State of California. These rules also specify our patients' rights to view and obtain access to their own information.
In certain instances, your written authorization is required to share your PHI. In other cases - to provide healthcare services, obtain payment, or operate our hospitals effectively and efficiently - John Muir Health is permitted to use your PHI without authorization. Our Notice of Privacy Practices - available for download below - explains in detail your rights and how your health information may be used. You will also be offered a copy of the applicable Notice each time you are a patient in one of our facilities.

As a patient at one of our facilities, you will find that:
- Each time you receive a new healthcare service at one of our facilities, you will be asked to review and acknowledge receipt of a Notice of Privacy Practices (Notice). This Notice gives a complete description of how we use, disclose and protect your medical information and of your patient rights regarding your medical information.
- You will have a right to inspect and receive copies of your medical information.
- You may request to amend your records if you feel the information is incorrect or incomplete.
- You have a right to an accounting of certain disclosures of your information that we have made and a right to request restrictions of our use or disclosure of your medical information. For example, you have a right to request that we communicate with you about medical matters at an alternate address.
- For inpatients at John Muir Health, unless you object, we will include your name, room location, religious affiliation, and general condition in the hospital directory so we can route visitors and phone calls to you.
- You have a right to authorize other uses and disclosures of your medical information not covered by our Notice or the laws that apply to John Muir Health.
- If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services.
