Privacy Statement
If you are planning an upcoming service, maternity stay, procedure or surgery, you can complete the hospital pre-registration process online in just a few minutes.
Please be sure to have on hand relevant primary and secondary insurance information, the guarantor's personal and employment specifics, and emergency contact numbers. Next, select one service from the list below and get started!
Please note: On-line pre-registration is currently available for scheduled outpatient and inpatient services at our three medical center locations: John Muir Medical Center, Walnut Creek Campus, John Muir Medical Center, Concord Campus, and John Muir Outpatient Center, Brentwood. Online pre-registration for our other locations will be available in the near future. As you are completing the form, be sure to look for the "info" links, if you want further explanation about the information being requested.
Pre-Register for Hospital Services:
Pre-Register for Outpatient Services:
Online pre-registration is currently available for services at our three medical center sites (Walnut Creek, Concord, Brentwood).
- Outpatient Services This includes radiology tests, ultrasound exams, mammography or cardiology testing. Please note that your registration must be completed 24 hours in advance of your scheduled appointment.
- Procedures This includes angiography, heart catheritization, endoscopy, GI and Eye lab procedures. Please note that your registration must be completed at least four days in advance of your scheduled appointment.
- Surgery This includes all preoperatives tests, such as Lab, X-Ray and EKG. Please note that your registration must be completed within 5 days of your scheduled surgery.
