Alzheimer's Disease/Dementia
A licensed, experienced geriatric care coordinator is available to assist older adults, their families, physicians and other health care professionals with concerns related to aging and health. The coordinator will provide a comprehensive assessment of the social, psychological, medical, environmental and functional factors affecting the individual's independence and ability for self-care. A care plan addressing these concerns will be developed and services coordinated with the individual's physician. Counseling regarding aging issues, care at home, cost of care, placement options, and other community resources is also available by phone.

Senior Services staff can check prescription pricing on the Internet to find the very best value for your prescription medication purchases. Low-income seniors may qualify for assistance in obtaining medications through pharmaceutical company outreach programs.
Volunteer counselors offer assistance with Medicare and other health insurance issues. This program is sponsored by the California Department of Aging and the Contra Costa County Office on Aging. Please call the Senior Services office to schedule an appointment.

The Advanced Health Care Directive class is an opportunity to clarify your own choices and make sure they will be carried out should you become unable to make your own health care decisions.
