Who We Are

What is today the John Muir/Mt. Diablo Community Health Fund came into being in 1997 when the merger that created John Muir Health called for developing and funding an independent "community health benefit nonprofit corporation." Since our inception, we have been dedicated to supporting programs that improve the health of medically uninsured and underserved populations in central and east Contra Costa County.

The Community Health Fund is governed by an appointed, ten-member board of directors. John Muir Health, a not-for-profit health system, provides funding for our grant program and program-related operations. Through 2012, the board has approved spending over $20 million in the form of grants for health initiatives and program-related expenses. Over the years, our incorporated 501(c)(3) tax-exempt public charity status has enabled us to combine resources with The California Endowment, The California Wellness Foundation, Y & H Soda Foundation, Contra Costa Health Services, and Contra Costa County Employment & Human Services to fund several important health initiatives.
