Lifestyle Management / Transitions of Care / Health Systems Management

This is a 6 week rotation broken into 2 three week sections. During this rotation residents spend 4 half days a week in their ambulatory care clinic. The rest of the week is spent learning about lifestyle medicine, meeting with system leaders, and learning about transitions of care.

The lifestyle management component focuses on the 6 pillars of lifestyle management that are so important to our patients and ourselves: nutrition, sleep, exercise, social support, decreasing toxin intake, and dealing with stress. It is a combination of online learning and working with experts in the field.

The Transitions of Care experience includes on half day per week in our Post-discharge Clinic and one half day per week in the Comprehensive Care Clinic.

The Health Systems management component includes speaking with various healthcare leaders.

The supervisor for this rotation are Heidi Joshi, PsyD, and Aparna Vemuri, MD.
