Are you experiencing new or worsening onset of any of the following?

• Fever
• Chills (without vaccine or for more than 2 days after receiving COVID-19 vaccine)
• Shortness of breath or difficulty breathing
• Unexplained malaise/fatigue (without vaccine or for more than 2 days after receiving COVID-19 vaccine)
• Unexplained muscle body aches (without vaccine or for more than 2 days after receiving COVID-19 vaccine)
• Unexplained headache (without vaccine or for more than 2 days after receiving COVID-19 vaccine)
• Loss of taste or smell
• Unexplained sore throat
• Unexplained congestion or runny nose or cough
• Nausea or vomiting
• Diarrhea

Within the past 14 days, have you been exposed to a household member or close contact who has tested positive for COVID-19?
