John Muir Health offers a full spectrum of services to meet a variety of healthcare needs. Our highly skilled staff collaborates closely with you, your family and your doctor to help you achieve your health care goals.

Home Health Services' experienced and caring team includes Registered Nurses; physical, occupational, and speech therapists; medical social workers; dieticians; and Certified Home Health Aides.

Getting Started

Your doctor can order our services for you if you are homebound and have a skilled need or you have recently given birth. We offer home health services to individuals of all ages, from infants to seniors with a wide range of clinical needs who reside in Contra Costa and parts of Alameda and Solano counties.

In 2010, we celebrated receiving the distinguished Press Ganey Award for Statistical Improvements in Patient Satisfaction.

We start by working very closely with you and your family to assess the specific needs for education and care to help you recover. Next, we develop a plan with you to provide the best care possible in your own home.

Quality Care

You can trust that our team will provide you the highest quality care. We're very experienced in providing skilled care with compassion and in accommodating the many diverse needs of our patients. Our staff often develops strong relationships with our patients,  regularly receiving heartwarming letters of thanks from patients and families who were touched by their caring nature.

Specialty Clinicians

Home Health Services can accommodate a wide range of care needs. In addition to our skilled nurses, we have Wound Ostomy and Continence Nurses (WOCN) on staff—experts in wound and incontinence care. Additional nurses on staff hold other advanced degrees and specialty certifications in cardiology, oncology and diabetes.

Nutrition consultations with a registered dietitian are available to help you with diet and nutrition. Our Medical Social Workers assist you and your family with long-term care planning and access to community resources.

Maternal/Newborn/Pediatric Home Care

Maternal/Newborn/Pediatric Home Care is provided by a special team of registered nurses, including International Board Certified Lactation Consultants on staff. Our team works with you to give you the care, support and information you need to successfully meet the challenges of motherhood with emphasis on education on safety, feeding, growth and development. During the first visit, a nurse conducts a thorough assessment of mother and baby and conducts any blood tests the doctor has ordered. Follow-up visits may be scheduled as needed.

Rehabilitation Therapy

Our highly trained rehabilitation staff of physical, speech and occupational therapists will work with you to help you regain and maximize your independence and enjoy a higher quality of life. We advise and assist patients in obtaining all the necessary equipment and supplies for their recovery at home and educate the patient and family throughout the rehabilitative process.

Certified Home Health Aide Services

A Certified Home Health Aide (CHHA) may be ordered by your doctor to assist you with personal care. CHHAs perform hands-on patient care to meet your needs in accordance with the plan of care under the direction and supervision of one of our licensed professional clinicians. They also assist in maintaining a safe and clean environment. CHHAs are generally scheduled between the hours of 8:00 AM and 4:00 PM. Other hours will be considered upon request.

CHHA Duties may include:

  • Assists with bathing, dressing, and feeding.
  • Tub bath/shower only after supervision by the supervising clinician or determination by her/him that this is a safe procedure.
  • Temperature, pulse and respiration as directed by the supervising clinician.
  • Assists with home exercise program developed by the nurse or therapist.
  • Reports physical/emotional changes to nurse.
  • Light household duties as outlined per the Plan of Care.
  • Maintains safety and communicates issues.

Phlebotomy Services

Phlebotomy services are available to perform lab draws requested by your physician when you have current orders to be seen by one of our skilled home health services.

Patients currently using our home health services have access to our 24 hour advice line staffed by a Registered Nurse at the main number listed above.

Services are scheduled seven days a week according to the patient’s care plan.
