Complex Community Care Coordination

The Complex Community Care Coordination (CCCC) program provides intensive hospital and community case management for vulnerable patients who need connections to services. A Social Work Case Manager and LVN Case Manager work from the John Muir Medical Center, Concord and in the community via home visits and appointment accompaniment, to assist with the following services:

• Access to health care
• Access to transportation
• Assessment of safety and health issues at home
• Access to financial assistance and health insurance
• Assistance in accessing employment and training services
• Assistance in finding and adjusting to permanent housing
