John Muir Health focuses on identifying—and then helping you achieve—your rehabilitation goals. We treat individuals of all ages with injuries or health-related conditions that limit their abilities to move or perform functional activities as well as they would like in their daily lives.

John Muir Health Outpatient Rehabilitation Center, Pleasant Hill is located just East of Highway 680 between Concord and Walnut Creek at a three story brick building in Corporate Park, Hookston Square. The center is located on the first floor. There's ample free parking.

About Our Programs

Hand Therapy
This program provides rehabilitation not only for the hand but also for conditions affecting the wrist, elbow, and shoulder girdle.

Neurological Therapy
After you are referred by your doctor and thoroughly evaluated, a team of specialists will help set treatment goals for recovery. This team can be made up of many skilled professionals, including the neurologist/neurosurgeon, an occupational therapist, a physical therapist, and a speech/language therapist.

Physical Therapy
This therapy may help you improve or restore mobility, regain functional ability, and help you reduce pain. In some cases, it can serve as an alternative to surgery and/or pain medication.

Speech Therapy
Working with the full range of human communication and its disorders, speech-language pathologists evaluate and diagnose speech, language, cognitive-communication and swallowing disorders in individuals of all ages, from infants to the elderly.

Neuropsychology Services
Comprehensive consultation services, with expertise in the evaluation of neurological disorders including traumatic brain injury, stroke, brain tumors, movement disorders, dementia syndromes, and cognitive disorders secondary to substance abuse. We also address questions regarding patient decision-making capacity.

A useful packet of information for patients who are getting treatment at one of John Muir Health’s outpatient rehabilitation clinics and who have disorders or injuries related to the elbow, wrist, hand, or fingers.

Neurological Conditions Packet
A useful packet of information for patients with neurological disorders who are getting treatment at one of John Muir Health’s outpatient rehabilitation clinics, including those who have had a stroke, Parkinson's disease, or multiple sclerosis.

Pediatric New Patient Registration Packet
A useful packet of information for pediatric patients with conditions related to most body parts including the spine, legs, and shoulders.
