Home Health - Skilled Needs Services

John Muir Health offers home health services that assist homebound patients of all ages who live in Contra Costa and parts of Alameda and Solano counties. Our patients include those with certain cardiac, respiratory, wound/ostomy, diabetes, neurologic, pediatric, and medical/surgical care needs.

If you are homebound and have a skilled need, your doctor can order our services for you. They include skilled nursing; physical, occupational, and speech therapy; medical social work; nutrition counseling; and Certified Home Health Aide services. We work closely with you, your family and your doctor to provide the best care possible.

An exception to the homebound status is the Maternal/Newborn/Pediatric Home Care Program which is a unique service that provides assessment and care to the newborn and mother, and information/instruction to the family.

Your Assurance of Quality

In 2010, the Joint Commission, which regularly surveys every aspect of our patient care, recognized our home health services with the highest praise. We are Medicare-certified and a long-standing member of both the National Association of Home Care and the California Association of Health Care at Home.

We work closely with Health Services Advisory Group, a Quality Improvement Organization. The Agency participates in national home health comparison survey, called Home Health Compare. John Muir Health consistently compares favorably when compared to other Home Health agencies.

Patients currently using our home health services may call the main number 24 hours a day to speak with a Registered Nurse.

Services are scheduled seven days a week according to the patient’s care plan.
