Appropriate Patients to Refer for Evaluation
  • Must document a Medical Necessity
  • Must be fluent in English
    • Evaluations have a significant language component an individual is best served when the evaluation is completed in his/her primary language.
    • Our staff is happy to work with providers on referrals to various neuropsychologists who complete evaluations in other languages.
  • Age 18 and older
  • The patient must have a cognitive concern or diagnosis.
    • If concern is primarily psychiatric without concern for underlying cognitive deficit the patient should be referred to for mental health/behavioral health services such as psychiatry or psychotherapy.
    • If the individual has a comorbid psychiatric diagnosis and concern for cognitive deficit, it is best if the psychiatric condition is well controlled and medications are stable.
  • Not actively abusing mind altering substances
