Instant MyChart Activation
When you schedule an appointment with one of our providers, you will receive an email or text message with a link to sign up for MyChart.
If the sign up link is used within 20 minutes of receiving the text or email, you will complete a streamlined registration process requiring the following information:
- Username
- Date of birth
- Email address
- Password
- Security question and answer
If the sign up link is accessed after 20 minutes, you will be required to enter your date of birth and zip code first to confirm your identity.
You may also use an activation code to sign up for MyChart. An activation code is included in your After Visit Summary (AVS) or you may request one from your doctor's office.
