Transfer Center

When you need a different type of care than you’re currently receiving, it’s not always easy to know where you should go or how to get there. Our patient transfer services are designed to help you reach the most appropriate care as quickly and safely as possible.

The Transfer Center at John Muir Health makes it easy for doctors to arrange urgent and non-urgent transfers and admissions to John Muir Health hospitals. The service operates 24 hours every day.

How It Works

Your doctor calls the Transfer Center and provides your information. The Transfer Coordinator or Registered Nurse helps identify the most appropriate John Muir Health location and doctor specialty.

The Transfer Center helps the referring doctor quickly reach an on-call doctor at the new location to discuss your needs in detail. The necessary paperwork and medical documents are transferred to your new location. If required, the Transfer Center also helps arrange transportation.

How to transfer a patient
  1. ED physician calls the John Muir Health Regional Transfer Center at (925) 947-4488
  2. The Transfer Coordinator, a Registered Nurse, helps to identify the appropriate John Muir Health location and physician specialty
         • Facilitates physician-to-physician communication
         • Ensures medical information is transferred to the new location
  3. If required, the Transfer Center may also help to arrange patient transportation
